Each of the forms listed is presented in Microsoft Word format (except for the IRS Tax Filing Procedure, which is presented in Adobe Acrobat format). You need JavaScript enabled to view it. ) or faxed (503-297-3694) to the Alpha Pi Mu National Office after obtaining the necessary signatures. The one exception is the IRS Tax form (Form 990-N/e-Postcard) which must be submitted online to the IRS. The initiation certificates and keys will be shipped to your chapter after the initiation payment is received.
